When hosting a qualifier, please note that the forms below will be needed depending on the qualifying divisions you choose to host.
Once your event is completed, please send your results to:

Please make sure to include with your results the Excel sheet entitled USDDN-Results-Emails.
Fees are to be sent via PayPal to dmardinusddn@gmail.com. Results and fees are due 10 days after the completion of the qualifier.
